DIRECTIONS
Envision Laser Centers offer multiple location for our patients conveniance. We have an Altoona LASIK surgery facility and a Johnstown LASIK surgery center, where every step of LASIK can be completed from your free LASIK consultation, to your pre-op exam, to LASIK and PRK surgery and all of the way through your post-op exams. We also have a State College LASIK care center and a Latrobe LASIK care facility, which allows you to have all the pre and post-operative testing performed.
ALTOONA LASIK SURGERY CENTER:
1424 11th Ave, Altoona, PA 16601
(located in downtown Altoona in Mountain View Eye)
From the East and North:
Follow I-99 South/ US-220 South to EXIT 33 (17th Street).
Travel on 17th Street approximately 1.9 miles to 12th Avenue. You will take a right onto 12 Avenue to 14th Street. Take a right onto 14th St and go to 11th Avenue (at a stop sign). Take a right onto 11th avenue. Envision Laser Centers and Mountain View Eye Associates are located on the right. Free on-street parking is available.
From the West and South:
Follow I-99 North/ US-220 North to Exit 33 (17th Street).

JOHNSTOWN LASIK SURGERY CENTER:
1318 Eisenhower Blvd, Johnstown PA 15904
(located in Richland inside Ophthalmic Associates)
From the North and South:
From 219 take 56 East exit onto Scalp Avenue. Turn left onto Eisenhower Blvd and we are located on the right, inside the new Ophthalmic Associates surgery center.
From the East and West:
Follow 22 to 219 South. Take Scalp Ave exit (56 East).Turn right off ramp and go to the 3rd traffic light. Turn North (Left) on Eisenhower Blvd and we are located on the right, inside the new Ophthalmic Associates surgery center.
STATE COLLEGE LASIK CARE CENTER:
315 Colonnade Blvd, State College, PA 16803
(located inside Target Optical)
From North Atherton St:
Turn onto Lowes Blvd. Turn left onto Colonnade Blvd into Target Optical (Dr. Lindsay Miller's Office).
From 322:
Go South on Waddle Road. Turn right @ light onto Lowes Blvd. Turn right onto Colonnade Blvd into Target
LATROBE LASIK CARE CENTER:
1933 Dailey Ave, Latrobe, PA 15650
(located inside Latrobe Vision Center) Just off Lincoln Avenue, North of 982.
